Acknowledgment of Risk:
I, the undersigned participant, acknowledge that my participation in the Group Healing Workshop offered by Heart Path Intuitive Services (hereinafter referred to as "the Workshop") involves inherent
risks that could result in personal injury, emotional distress, illness, or damage to personal property. I understand that these risks may include, but are not limited to:
Physical and Emotional Healing: The healing practices and modalities used in the Workshop may bring up intense emotions, physical sensations, or other unexpected reactions that could be uncomfortable or distressing.
Medical Conditions: Participation in the Workshop may impact pre-existing medical
conditions or result in new health issues.
Property Damage: Personal property may be lost, stolen, or damaged during the Workshop.
Assumption of Risk:
I fully understand and voluntarily assume all risks associated with my participation in the Workshop,
whether they are known or unknown, foreseeable or unforeseeable.
Release of Liability:
In consideration of being allowed to participate in the Workshop, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue
Heart Path Intuitive Services, its owners, facilitators, employees, agents, volunteers, and any other associated parties (hereinafter collectively referred to as "the Released Parties") from any and all
liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, illness, emotional distress, or death that may be sustained by me, or to any property belonging to me, whether caused by the negligence of the Released Parties or otherwise, while participating in the Workshop.
Indemnification:
I agree to indemnify and hold harmless the Released Parties from any and all claims, damages, costs, and expenses, including attorneys' fees, arising from or related to my participation in the Workshop, including any claims brought by third parties related to my actions during the Workshop.
Medical Treatment:
I hereby authorize the Released Parties to seek emergency medical treatment on my behalf if deemed necessary, and I agree to assume full responsibility for any costs associated with such treatment.
Media Release:
I understand that photographs, video recordings, and other media may be taken during the Workshop. I hereby grant Heart Path Intuitive Services and its representatives the right to take, use, and publish images, video footage, and audio recordings of me, in whole or in part, in any manner
and for any lawful purpose, including, but not limited to, promotional materials, advertising, social media, and other forms of publicity for future events or workshops. I understand that these materials
may be used without further notice, permission, or compensation.I waive any right to inspect or approve the final products, including written or electronic copy, wherein my likeness appears. I release the Released Parties from any and all liability that may arise from the use of these images and recordings.
Governing Law:
This Release of Liability Waiver and Media Release shall be governed by and construed in accordance with the laws of the [State/Country] in which the Workshop takes place, without regard to its conflict of laws principles.
Severability:
If any provision of this waiver is found to be unenforceable or invalid, that provision shall be severed from this agreement, and the remainder of the waiver shall remain in full force and effect.
Acknowledgment of Understanding:
I have read this Release of Liability Waiver and Media Release and fully understand its terms. I acknowledge that I am signing this waiver freely and voluntarily, and I intend my signature to be a
complete and unconditional release of all liability to the greatest extent allowed by law.
